Consent of Broadcast and Advertising

I agree and consent to the recording and broadcast of my voice, name, and/or likeness by Richard Harris Law Firm. This consent includes any promotional or advertising material that Richard Harris Law Firm, in its sole discretion, determines to be most appropriate. Richard Harris Law Firm may use my name, likeness, voice, or words in any promotional and advertising material including, but not limited to, television, radio, print, direct mail or Internet.

I acknowledge that:
• I am under eighteen (18) years of age, and my parent or legal guardian’s initials below give me consent to participate in the Richard Harris Law Firm PSA contest and give me consent for all the above listed stipulations.
OR
• I am older than eighteen (18) years of age and I have the right to enter into this consent agreement.

I acknowledge that I will not receive any compensation now or in the future. I consent that Richard Harris Law Firm shall have the right to use my name, likeness, voice, or words for an indefinite duration. I understand that I am relinquishing any personal or property right which I might otherwise have in the subject matter of this consent agreement and I am releasing Richard Harris Law Firm and any and all of its agents, representatives, or clients from any violation of any personal or property right which I might otherwise have in the subject matter of this consent form.
